The ‘Contact Us’ page has our contact and address details and a map.
The ‘Forms’ page has an order form for you to order your ostomy supplies, a form for changes of details (address, marital status, reversals or deceased). There are also forms to be used by Doctors and/ or Stomal Therapy Nurses.
The ‘Links’ page has links to the suppliers of Ostomy Appliances, and a link to the Australian Council of Stoma Associations (ACSA) from which you can access many Australian Ostomy Association sites, plus sites overseas.
